Please confirm the annual income thresholds and other requirements at the following website: https://www.pwc-kenpo.jp/eng/member/outline/family_a.html. Once your spouse no longer satisfies these requirements, promptly use the PC provided by PwC to access the HC Web, go to the family change menu, change your spouse’s social insurance status to indicate that they are not a dependent, and then submit this change. You will receive instructions from the PwC contractor concerning the documents* that need to be submitted.
Please prepare these documents and send them to the contractor (*a notice of dependent transfer and Health Insurance Eligibility Certificate (a valid document assuming you received a paper copy) (original) as well as a copy of the health insurance eligibility certificate for the new health insurance society, etc., if any, or a copy of some other document that can be used to confirm the date of qualification (not the date of issue) such as notice of eligibility information).
In addition, if your spouse does not intend to enroll in their employer’s social insurance, please bring the Certificate of Loss of Eligibility issued by the Health Insurance Society to the municipality’s National Health Insurance service counter and complete the enrollment procedure.
