Please use the PC provided by PwC to access the HC Web, go to the family change menu, change your spouse’s social insurance status to indicate that they are not a dependent, and then submit this change. You will receive instructions from the PwC contractor concerning the documents* that need to be submitted. Please prepare these documents and send them to the contractor. (*Notice of dependent transfer, Employment Insurance Eligibility Certificate (copies of both sides), your spouse’s Health Insurance Eligibility Certificate, etc. (if you received a paper copy) (original))
After we receive your documents, we will issue a “Certificate of Loss of Eligibility,” so please present this at your municipal office and complete the procedure to enroll them in National Health Insurance.
