Please fill out the designated items on the “Application Form for Medical Care Expenses,” attach the medical cost details (original)* and receipt (original) issued by the medical institution, and submit them to the Health Insurance Society. The Health Insurance Society will examine this documentation based on the relevant standards and then pay the appropriate amount.

* Medical cost details are not ordinarily issued and must instead be obtained by separately requesting them from the medical institution. Note that you are responsible for paying the costs of creating this document, if any.
