Please send a “Notice of Disqualification for Voluntary Continued Coverage Status” to the Health Insurance Society, and be sure to attach Health Insurance Eligibility Certificates for every member of your family (valid documents assuming you received paper copies) (original) as well as a copy of the health insurance eligibility certificate for your new health insurance society, etc. or a copy of some other document that can be used to confirm your date of qualification (not the date of issue) such as notice of eligibility information.
In the case of insurance-premium reimbursement due to advance payment, etc., the reimbursement will be transferred to the designated account by the end of the month following the month when the loss procedure was completed.
