Please note that the available methods vary depending on the status of information linkage with the PwC Health Insurance Society. Select the option that applies to your situation.

1. 1. If you have already submitted your Individual Number (My Number) to your employer and the linkage to your health insurance eligibility has been completed.
   (Linkage is processed approximately once every 10 days.)

2. For anyone (including those who have not submitted their My Number to their employer, or whose linkage is still in progress)
   Note：The dependent’s Code and Number are identical to those of the insured person.
