Health insurance cards are no longer newly issued or reissued since December 2, 2024. Use your My Number Card as your Health Insurance Certificate.When the My Number Health Insurance Card cannot be used, please obtain and use the digital eligibility certificate via KOSMO Communication Web(KOSMO).

Insured persons and dependents who wish to have a paper-based Health Insurance Eligibility Certificate issued (reissued) for the following
<Criteria>
Due to reasons such as requiring long-term care, having a disability, residing in a facility, or being of advanced age (65 years or older) it is difficult for the individual to use either the My Number Health Insurance Card or the digital Eligibility Verification Certificate when visiting medical institutions. In the event that the Health Insurance Society requests verification regarding this matter, an explanation can be provided without delay.

<Notes: Applications based on the following reasons will not be accepted>

  • 1. Wishing to keep a paper-based Eligibility Certificate just in case
  • 2. Being currently unable to use the My Number Health Insurance Card
  • 3. Not wanting to use the digital eligibility verification certificate
  • 4. For submission in case of a child’s nursery or school-related medical visit
  • 5. The individual is an underage person.

Notes: If you have a valid Paper-based Certificate of Health Insurance Eligibility at the time your eligibility is lost (for example, due to transfer to another LoS at PwC Japan, resignation, or removal from dependent status), you are required to return the certificate to the Health Insurance Society without delay. Please note that the shipping cost for returning the certificate must be borne by you.
