For persons residing outside of Japan and persons planning to move out of Japan (such as those assigned overseas)

Please take steps in advance so that your health insurance will apply if you go to a medical institution while you are temporarily back in Japan.

①For persons planning to move out of Japan

Please respond with "Yes" or "No" depending on your situation, and then take steps【A】to【D】below.

Do you have an Individual Number Card?
Can you use your Individual Number Card as a Myna health insurance card?(※)

【A】

Complete the procedure for changing to an expatriate Individual Number Card.

【B】

Link your Individual Number Card to your health insurance card (and then proceed to [A]).
Can you get an Individual Number Card before your departure?

【C】

Get your Individual Number Card, and then link it to your health insurance card (and then proceed to [A]).

【D】

Apply for an Individual Number Card after departing from the country.

（※）Requirements for using your Individual Number Card as a Myna health insurance card

  • Your Individual Number Card must be linked to your health insurance card.
    (Even if your electronic certificate has expired, your card can still be used for three full months following the last day of the month that includes the expiration date. For example, if the expiration date is March 3, your Myna health insurance card is usable until June 30).

【A】Complete the procedure for changing to an expatriate Individual Number Card.

If a Japanese citizen completes the procedure for continuing to use their Individual Number Card by the day before they plan to move out of Japan at their local government service counter, they can continue to use their Myna health insurance card even after moving out of Japan.
Completing this procedure also enables Japanese citizens to immediately go to medical institutions when temporarily returning to Japan.

【B】Get your Individual Number Card, and then link it to your health insurance card.

  • Submitting your Individual Number to PwC Japan

The PwC Health Insurance Society regularly obtains all members' Individual Numbers from PwC Japan and links them to insurance eligibility.

【C】Get your Individual Number Card, and then link it to your health insurance card.

【D】Apply for an Individual Number Card after departing from the country.

It has become possible for persons who departed from Japan on October 5, 2015, or later and do not have an Individual Number Card to apply to have a new card issued from outside of Japan as well.

②For persons currently residing outside of Japan

Please respond with "Yes" or "No" depending on your situation, and then take steps【E】to【G】below.

Did you depart from Japan on or later than October 5, 2015?
Do you have an expatriate Individual Number Card?
Can you use your Individual Number Card as a Myna health insurance card? (※)
No action required (Please use your Myna health insurance card at medical institutions.)

Link your Individual Number Card to your health insurance card upon returning to Japan.

【F】

Create an expatriate Individual Number Card.

【E】

Apply for an Individual Number Card upon returning to Japan.

No Individual Number has been assigned to you. When you transfer to Japan, a certificate of residence will be created for you and an Individual Number will be assigned to you. Later, after your Individual Number Card is created, if you link it to your health insurance card, you can use it as a Myna health insurance card.

Present your Myna health insurance card at a medical institution.

【J】

Link your Individual Number Card to your health insurance card while in Japan.

Apply for a KOSMO ID, which is necessary for the issuance of an electronic Health Insurance Eligibility Certificate.

【I】Present your Myna health insurance card at a medical institution.

You can use your Individual Number Card directly as a Myna health insurance card.

【J】Link your Individual Number Card to your health insurance card while in Japan.
